Meet our new Silver Chain liaison nurse
Cheryl Pappas has joined palliative care providers Silver Chain as the hospital liaison nurse for WSLHD.
WSLHD partnered with Silver Chain this month to give patients the option of enhanced expert palliative care services in the comfort of their own home.
Services include assistance with daily living, respite care and bereavement support.
Cheryl previously work at Auburn Hospital and in the community as a palliative care nurse.
In her new role, she will:
- Assist with forward discharge planning of patients who are receiving Silver Chain services or will be referred to Silver Chain.
- Visit clients in hospital, and then at home for continuity of care
- Help facilitate the smooth transition of Silver Chain community clients to and from the hospital palliative care units.
